Sample Format for Organization Charts
Organization Charts Procedure
Organization charts are maintained to outline the formal structure and functional areas of an organization and to define the reporting hierarchy of an organization.
This procedure outlines the requirement of maintaining organization charts within Integrated Solutions.
The Project Specialist is responsible for the distribution/notification of any new organization chart. This can be hard copy or electronic. Quarterly the Project Specialist for the CEO will print. have it signed by the CEO, distributed to all employees and file a hard copy of the complete organization chart.
The latest version is always available on-line and controlled by the CEO’s Project Specialist.
Recommendations for changes to the organization chart are made anyone in the organization, but must be approved by the CEO. Once approved, the CEO’s Project Specialist will make the changes, have it signed by the CEO, distributed to all employees and file the original.
Charts include: functions in the organization, each employee's name, date and manager’s signature (hard copy only).
The following is a sample format for organization charts, but strict adherence to this format is not required:
